A Guide on The Neurocare Consultation

Movement, breathing, senses and thought all depend on the brain and central nervous system. We strive to improve patient outcomes by combining classic psychotherapy with Transcranial Magnetic Stimulation (TMS), improving network connectivity in the brain.

The study is a non-blinded, randomised controlled trial testing NeuroCARE versus usual care. The intervention is based on cognitive-behavioural theory and includes training in mindfulness, coping skills and assertiveness techniques. Caregivers complete study assessments at baseline, 11 weeks and 16 weeks postrandomisation.

Assessments

Neuropsychological evaluations help determine a diagnosis, differentiate between conditions with similar presentations (such as traumatic brain injury and degenerative dementias), provide insight into treatment response and help predict long-term outcomes. They also help develop plans for future treatment like occupational and speech therapy.

During an assessment, the patient will answer questions and work with a trained technician on paper-and-pencil tests that are often very similar to games or puzzles. These tests are designed to measure different cognitive functions, such as memory, attention, and language.

The best time to conduct assessments is when a patient is alert and following commands. This is when you can really pinpoint any changes in their neurological status, such as whether they are becoming more confused or if their balance has gone out of whack because of medication side effects.

Case Management

Case management is a broad term describing advocacy, assessment, communication, service facilitation and resource management practices in health care, social work, and related disciplines. There are a number of different case management models, which are broadly defined by the principles of autonomy, beneficence, nonmaleficence, and veracity.

The planning phase involves identifying specific goals, care objectives and actions (both short- and long-term) in order to meet the client’s needs identified in the screening and assessing phases. This includes establishing and sharing key information on an ongoing basis with the client, their support system and healthcare providers/clinicians as well as community-based agency representatives.

The monitoring phase is related to assessing how the case management plan is working in order to determine whether the outcomes/goals have been achieved. This often includes requesting ongoing feedback and conducting evaluations on an ad hoc basis or at milestones. It also entails making sure that the information shared is accurate and complete.

IMEs

In some states, workers’ compensation judges rely heavily on IME (independent medical examination) doctors to evaluate the relationship between a worker’s disability and their work-related injury. In a workers’ comp case, an IME doctor will review the relevant medical records and perform a physical exam.

Workers interviewed for this report have reported that IME doctors give their one-sided reports equal or greater weight than the comprehensive evaluations of a worker’s treating physicians. Often, one-sided IME decisions are used to deny, diminish or delay benefits, such as wage-replacement or medical treatment.

In addition, IME doctors are not bound by patient confidentiality, so information about your daily activities may be used against you. If you notice a factual mistake in an IME report, you should make a note of it as soon as possible and notify the doctor. Having a written record will help you and your attorney challenge the IME report. In some states, you can request that an independent medical examination be conducted by a physician of your choosing.
